The Transportation Safety Board of Canada (TSB) is an independent agency created to advance transportation safety through the investigation of occurrences in the marine, pipeline, rail and air modes of transportation.
Advancing transportation safety
At the TSB, our mandate is to advance transportation safety by conducting independent investigations into selected transportation occurrences, whether marine, pipeline, rail, or aviation. That means finding out what happened, why it happened, and what needs to be done to prevent it from happening again.

The Transportation Safety Board of Canada is issuing four recommendations to Transport Canada and releasing its investigation report (A21C0038) which determined that insufficient regulatory requirements and defences to protect against loss of visual reference accidents led to the fatal 2021 crash of an Airbus AS350 helicopter on Griffith Island, Nunavut. Risk of Collisions from Runway Incursions
Переглядів 738Рік тому
Every year, there are millions of successful takeoffs and landings on Canadian runways. However, an accident can occur when an aircraft or vehicle incurs upon a runway surface that is actively in use. The risk of collisions from runway incursions is an issue on Watchlist 2022.
Fatigue Management
Переглядів 969Рік тому
In the transportation industry, crews often work long and irregular schedules that are not always conducive to proper restorative sleep. Fatigue poses a risk to the safety of air, marine, and freight train operations because of its potential to degrade several aspects of human performance. Fatigue management is an issue on Watchlist 2022.
Commercial Fishing Safety
Переглядів 387Рік тому
Despite various initiatives that have sparked the development of a safety culture within the commercial fishing industry, the same safety deficiencies on board fishing vessels continue to put at risk the lives of thousands of Canadian fish harvesters, and affect their families and communities’ well-being. Commercial fishing safety is an issue on Watchlist 2022.
Safety Management
Переглядів 367Рік тому
Some transportation operators in the air, marine and rail sectors are not managing their safety risks effectively, and many are still not required to have formal safety management processes in place. Moreover, those operators that have implemented a formal safety management system (SMS) are not always able to demonstrate that it is working and producing the expected safety improvements. Safety ...
Unplanned/Uncontrolled Movement of Rail Equipment
Переглядів 676Рік тому
Despite significant safety action taken by Transport Canada and the railway industry since the Lac-Mégantic accident to reduce the number of unplanned/uncontrolled movements of rail equipment, the number of occurrences has continued to trend upwards, posing a significant risk to the rail transportation system. Unplanned/uncontrolled movement of rail equipment is an issue on Watchlist 2022.
Regulatory Surveillance
Переглядів 366Рік тому
Regulatory surveillance has not always proven effective at verifying whether operators are, or have become, compliant with regulations and able to manage the safety of their operations. Furthermore, Transport Canada hasn’t always intervened on a timely basis to ensure transportation operators in the air, marine and rail sectors take appropriate corrective actions. Regulatory surveillance is an ...
Following Railway Signal Indications
Переглядів 389Рік тому
For over a century, Canada has relied on a system of visual signals to control traffic on a significant portion of its rail network. Sometimes, however, train crews misinterpret or misperceive a signal indication, which results in it not being followed. In the absence of physical fail-safe defences, this could result in a collision or a derailment. Following railway signal indications is an iss...

So the propeller still going would scare the shit out of me I wonder if they could hear the blades moving
My grandfather died on a foss boat a long time ago in BC. Lawrence Berg. RIP. It's amazing to me that my great grandfather was one of the founder's of this awesome company.
12 yrs later the signal system is just the same as it was
I find it funny that the type of locomotive was used was used in a movie called atomic train and its on the same route too
The Poseidon Adventure low bugget:
as the biggist fan this made me so sad
Money was spent. People may have been fired.
Is the train actually pulling only a center beam flatcar?
Good animation but it happened in the dead of night. "Queen of the North sank after running aground on Gil Island in Wright Sound, 135 kilometres (73 nmi) south of Prince Rupert, British Columbia. She sank at 12:25 am or 12:43 am PST (08:43 UTC) on March 22, 2006; there are conflicting reports about the exact time." - en.wikipedia.org/wiki/MV_Queen_of_the_North#:~:text=On%20March%2022%2C%202006%2C%20with,found%2C%20died%20in%20the%20incident.
Typical Canadian boat captain...lol
Went down quicker than a brides nightie.
Кораблики снюхались
попали в водоворот, бывает
From ship engine, to airplane engine.
holly shit 2 guys went under ship, and one who got out from inside boat just in time than water rushes in
Стала подводная лодка
Guy running the deck with no PFD !!! WTF ??
Turned turtle, and the engines are still running!
what happened to Brody after the end of Jaws Unleashed
Gotta hand to the captain (?) for going down with the ship in such an heroic manner.
Not really. Airplanes use propellers all the time.😅
It probably just identifies as a hovercraft
Surprised how long the props spun. Engines running upside down.
The navigator went to jail for a few years. After that BC Ferries had two Navigators on duty on the Northern routes. There also should have been alarms if the ship missed it's turn.
Beautiful spot for such a terrible thing.
Anybody know the method of signalling in Canada?? Is it track circuit block, set by signallers? or token block working?I’m a signaller in Ireland and have been wanting to find out more info about Canadian railway jobs in signalling? I don’t think the Canadians call us guys signallers, perhaps they have a different name for them? yes We have other safety features like TPWS, AWS and SPAD indicators to prevent trains proceeding without authority passed signals at danger.
1:07 train wheels: goodbye forever
I wasent on this bc ferries but I have been on other ones and this is crazy
Was it a sail boat?
guy was raiing his girlfriend on the bridge and wasnt paying attention lol idiot happened right outside hartley bay
What Was The Number Of This Locomotive?
9606, it’s in the report in the description
0:20 THERES A HAND COMING OUT THE TUGBOAT
Dangerous
They are going to sue CN for this
in little Portugal toronto ontario canada
What engine number?
Look in the description
Was that a dog that fell back into the tugs bridge?
I think the engine was a runaway
The ship was headed for shore too long. Only a matter of time the vessel would strike. Clearly the crew at the helm were not paying attention. If they had, The Queen of the North would probably still be sailing and those who perished would be alive today.
My SOBRIETY sponsor-one hell of a (sober) guy….. WAS the Chief ENGINEER on board that night…….. Testifying in B.C. Court as the main antagonist to the BITCH ASS captain. He told me… to my face……..WHAT ACTUALLY happened that night.
Pov:you were on that boat what ate you going to do? A:evacuate b:Wait until the rescue boat comes(will sink)
CORRECT TITLE TO VIDEO- “TRUCK STALLED ON RR TRACK CAUSES DERAILMENT “!!
Sechelt Rapids has a very tight window to be used for ANY boat movement. There's a cement operation there running 24/7 on the non-road north side. I used to rent the fourth cabin on the north road side from a buddy. When the rapids start to run you do NOT have time to react; looks like the barge made that decision and it's all over fast.
Is this the one that the crew was frigging in the rigging, AKA screwing and partying?
As a former locomotive engineer and instructor, I wholeheartedly endorse the recommendations of the TSB and associated regulatory standards.
I have an idea that will definitely solve this problem. It is safe, it will contribute a lot, it will save lives, it will stop having any losses.
Glad to hear there was no fatalities, but I noticed not one of the crewmen had a PFD on.
The engineers I think should have been notified of the work being done on the tracks ahead of time and they should have paid attention to the signals
reason is crash: speeding
Surely they would have been better running the ship aground
